- A copy of the civil status ID or the unified card number 1

Receipt from our college accounts
A service continuity letter from (exclusively the Department of Health) + the undertaking to complete the medical apprenticeship period + a copy of the guarantee guarantee.
To obtain a graduation endorsement, please bring the following:

One copy of the Civil Status ID or the unified card
-Written request
- Service continuity letter from (Department of Health exclusively).
